A 22 year old male presents with a history of sudden onset left sided pleuritic chest pain. There is no past history of note and he is not on any medication.

On examination he has a pulse rate of 100 beats per minute his BP is 110/70. On auscultation of his heart you hear a crunching sound in time with his heartbeat.
