A 72 year old male presents with a history of feeling weak and lethargic, he also complained of loss of weight and feeling nauseated and vomiting.

On examination his skin was pigmented and there was pigmentation of his palmar creases. BP 100/60 lying 70/50 standing.

Investigations

Sodium 122 mmol/L

Potassium 5.4 mmol/L

Urea 17 mmol/L

Bicarbonate 14 mmol/L

Blood glucose 4.1mmol/L
