You are called in the night
to see a patient who has become agitated on the orthopaedic ward. The patient
had been brought in to casualty by the police having been found lying on the
road. He had suffered a fracture of his humerus and the orthopaedic surgeons
had managed him with a sling but they had been unable to discharge him because
there were no social services available.
The history in the notes was
sketchy as the patient had been uncooperative and there was no mention of past
illnesses drugs or alcohol history or history of drug abuse.
On examination of the
patient you note that he is agitated and confused. He keeps complaining that
there are animals and insects attacking him. You are unable to examine him
formally but you note that he is tremulous.
